Job Description

The Registered Nurse Case Manager is responsible for planning, coordinating and facilitating medical services for patients. It is important that Registered Nurse Case Managers focus on high quality medical services and efficient processes for the patient and organization.

Responsibilities

A Registered Nurse Case Manager's responsibilities include:
  • Talking with the patient and their family to identify their needs.
  • Establishing approach to satisfy the patient’s needs.
  • Providing resources to the patient and their family members.
  • Help in locating the most affordable method of care.
  • Ensuring the plan is meeting and exceeding the patient’s needs.

Requirements

One year of experience as a RN Case Manager within the past two years.
  • Current RN License in good standing.
  • Current BLS through American Heart Association.
  • Must pass pre-employment drug screen.
