The RN Case Manager is responsible for planning, coordinating and facilitating service for patients. It is vital that RN Case Managers focus on high quality medical services and efficient processes for the patient and organization.

Responsibilities

A Registered Nurse Case Manager's responsibilities consist of:
  • Meeting with the patient and their family to diagnose their needs.
  • Creating a plan to satisfy the needs of the patient.
  • Providing resources to the patient and their family members.
  • Assisting in locating the lowest cost method of care.
  • Making sure the plan is meeting the patient's needs.

Requirements

One year of experience as a RN Case Manager within the past two years.
  • Current Nursing License in good standing.
  • Up to date Basic Life Support through American Heart Association.
  • Negative Drug Screen.
