Job Description

The RN Case Manager is responsible for planning, coordinating and facilitating necessary services for patients. It is crucial that Registered Nurse Case Managers focus on providing quality medical services and efficient processes for the patient and organization.

Responsibilities

A RN Case Manager’s responsibilities consist of:


  • Meeting with the patient and their family to determine their needs.

  • Establishing strategies to satisfy the needs of the patient.

  • Providing resources to the patient and their family members.

  • Assisting in locating the lowest cost method of care.

  • Following up to ensure the plan is meeting and exceeding the patient’s needs.

Requirements

One year of experience as a Registered Nurse Case Manager within the past two years.


  • Current RN License in good standing.

  • Current BLS from American Heart Association.

  • Negative Drug Screen.
