Job Description

The Registered Nurse Case Manager is responsible for planning, coordinating and facilitating service for patients. It is vital that Registered Nurse Case Managers focus on providing quality medical services and efficient processes for the patient and organization.

Responsibilities

A Registered Nurse Case Manager's duties include:
  • Communicating with the patient and their family members to diagnose their needs.
  • Developing approach to satisfy the needs of the patient.
  • Providing services to the patient and their family members.
  • Assisting in locating the lowest cost method of care.
  • Ensuring the plan is meeting and exceeding the needs of the patient.

Requirements

One year of experience as a RN Case Manager within the past two years.
  • Current RN License in good standing.
  • Up to date BLS through American Heart Association.
  • Must pass pre-employment drug screen.
