The Registered Nurse Case Manager is responsible for planning, coordinating and facilitating medical care for a patient. It is crucial that Registered Nurse Case Managers focus on high quality medical services and efficient processes for the patient and organization.

Responsibilities

A RN Case Manager's responsibilities include:
  • Communicating with the patient and their family to determine their needs.
  • Creating a plan to satisfy the patient's needs.
  • Providing services to the patient and their family members.
  • Assisting in locating the most affordable method of care.
  • Ensuring the plan is exceeding the patient's needs.

Requirements

One year of experience as a Registered Nurse Case Manager within the past two years.
  • Current RN License in good standing.
  • Current BLS from American Heart Association.
  • Negative Drug Screen.
