Job Description

The Registered Nurse Case Manager is responsible for planning, coordinating and facilitating necessary services for patients. It is important that Registered Nurse Case Managers focus on providing quality medical services and efficient processes for the organization and patient.

Responsibilities

A RN Case Manager’s duties include:


  • Meeting with the patient and their family to determine their needs.

  • Establishing strategies to satisfy the needs of the patient.

  • Providing support to the patient and their family members.

  • Help in locating the lowest cost method of care.

  • Making sure the plan is meeting and exceeding the patient’s needs.

Requirements

One year of experience as a Registered Nurse Case Manager within the past two years.


  • Active Nursing License in good standing.

  • Up to date BLS through American Heart Association.

  • Must pass pre-employment drug screen.
