Job Description

The Registered Nurse Case Manager is responsible for planning, coordinating and facilitating service for a patient. It is vital that RN Case Managers focus on high quality medical services and efficient processes for the organization and patient.

Responsibilities

A RN Case Manager's duties consist of:
  • Meeting with the patient and their family to determine their needs.
  • Establishing strategies to satisfy the patient’s needs.
  • Providing assistance to the patient and their family members.
  • Assisting in locating the most cost-effective method of care.
  • Following up to ensure the plan is meeting the patient’s needs.

Requirements

One year of experience as a Registered Nurse Case Manager within the past two years.
  • Active RN License in good standing.
  • Up to date Basic Life Support from American Heart Association.
  • Must pass pre-employment drug screen.
