Job Description

The RN Case Manager is responsible for planning, coordinating and facilitating medical services for a patient. It is vital that RN Case Managers focus on providing quality medical services and efficient processes for the organization and patient.

Responsibilities

A Registered Nurse Case Manager's duties consist of:
  • Meeting with the patient and their family to identify their needs.
  • Establishing strategies to satisfy the needs of the patient.
  • Providing assistance to the patient and their family.
  • Help in locating the most cost-effective method of care.
  • Reaching out to ensure the plan is exceeding the patient’s needs.

Requirements

One year of experience as a RN Case Manager within the past two years.
  • Active Nursing License in good standing.
  • Up to date BLS from American Heart Association.
  • Negative Drug Screen.
