Job Description

The Registered Nurse Case Manager is responsible for planning, coordinating and facilitating necessary services for a patient. It is important that Registered Nurse Case Managers focus on providing quality medical services and efficient processes for the organization and patient.

Responsibilities

A Registered Nurse Case Manager’s responsibilities consist of:


  • Talking with the patient and their family to determine their needs.

  • Developing approach to satisfy the patient’s needs.

  • Providing support to the patient and their family.

  • Help in locating the most cost-effective method of care.

  • Following up to ensure the plan is meeting the needs of the patient.

Requirements

One year of experience as a Registered Nurse Case Manager within the past two years.


  • Active Nursing License in good standing.

  • Current Basic Life Support from American Heart Association.

  • Must pass pre-employment drug screen.
